Introducing ETRAISI: Endovascular Thrombolysis/Thrombectomy Registry in Acute Ischemic Stroke of Iran – A Methodology Protocol

INTRODUCTION: Stroke is a leading cause of mortality and long-term disability worldwide and also has a substantial burden in Iran. Acute ischemic stroke (AIS) accounts for most cases, and patients' outcomes are highly affected by timely acute care and reperfusion therapies. However, national data on stroke care processes and outcomes remain fragmented. Thus, establishing a standardized stroke registry is essential to monitor real-world practice, evaluate quality metrics, and support research and healthcare policies. METHODS: ETRAISI (Endovascular Thrombolysis/Thrombectomy Registry in Acute Ischemic Stroke of Iran) is a prospective, quality-of-care, hospital-based registry designed to provide a sustainable data infrastructure for adult AIS patients eligible for intravenous thrombolysis (within 4.5 h of onset)/mechanical thrombectomy (within 6 h of onset). Standardized data across six domains, including demographics, clinical characteristics, stroke characteristics and workup results, time metrics, treatments, and outcomes, are collected from admission through discharge and 3-month follow-up using a secure web-based platform. Data quality is ensured through predefined validation rules, audits, and centralized oversight, with ethical approval and informed consent obtained for all participants. CONCLUSION: By systematically recording detailed workflow processes, treatment, and follow-up outcomes, ETRAISI addresses gaps in stroke data and enables quality improvement, research, and evidence-based policy-making in a resource-limited setting. The registry provides a scalable national framework for optimizing stroke care and strengthening stroke surveillance.
