PROSTHODONTIC REHABILITATION IN PATIENTS WITH BRUXISM: CLINICAL CHALLENGES AND THERAPEUTIC STRATEGIES

Bruxism is a masticatory muscle activity characterized by clenching, grinding of the teeth, or mandibular bracing, which may occur during sleep or wakefulness. Due to its potential to cause tooth wear, restorative fractures, muscular alterations, and functional impairment of the stomatognathic system, this condition represents a significant challenge for restorative and rehabilitative dentistry. The present study aimed to review the scientific literature regarding the main clinical challenges involved in prosthodontic rehabilitation of patients with bruxism and to analyze the therapeutic strategies employed to increase treatment longevity. This narrative literature review was conducted through searches in the PubMed/MEDLINE, SciELO, Google Scholar, and ScienceDirect databases, including studies published between 2016 and 2026. The findings demonstrated that patients with bruxism are at greater risk of severe tooth wear, mechanical complications in restorations and prostheses, and increased susceptibility to failures in implant-supported rehabilitations. The literature also highlights the importance of careful selection of restorative materials, appropriate occlusal planning, the use of stabilization occlusal splints, and periodic clinical follow-up as key strategies to minimize the effects of parafunctional activity. It can be concluded that a multidisciplinary and individualized approach is essential to promote greater clinical predictability, functional stability, and longevity of prosthodontic rehabilitations in patients with bruxism.
