Clinical Usefulness of 8-F Sheathless Guiding Catheter for Transradial Approach in Complex Percutaneous Coronary Intervention

This study evaluated the safety and efficacy of complex transradial coronary intervention (TRI) with an 8-F sheathless guiding catheter (GC).We enrolled 106 consecutive patients who underwent complex transradial coronary intervention (TRI) using 8-F sheathless GC at multiple centers between August 2021 and December 2022 and assessed the clinical effects and incidence rates of early radial artery occlusion (RAO) after the TRI. Ultrasound assessment for the target radial access was performed both before (following administration of nicorandil) and after the procedure. All patients successfully underwent the TRI. Among the patients, 3 (3.1%) developed RAO the day after the procedure, whereas 7 (6.6%) developed RAO within 3 months. Nicorandil administration significantly increased the inner diameter of the radial artery (before: 2.80 ± 0.49 mm, after: 3.00 ± 0.56 mm). TRI with an 8-F sheathless GC might be effective for patients with high bleeding risk who require complex percutaneous coronary intervention procedures.
