The interplay between the obsessive-compulsive disorder and sexual function: a systematic review

INTRODUCTION: This review explores the relationship between obsessive-compulsive disorder (OCD) and sexual functioning. OBJECTIVES: The aim is to highlight the clinical relevance of sexual health in OCD and encourage more comprehensive assessment and intervention strategies. METHODS: This study explores the association between OCD symptoms and genital pain or sexual functioning, using the Desire, Excitement, Plateau, Orgasm, Resolution model as a conceptual framework. We conducted a Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020-compliant systematic review, searching PsycINFO, MEDLINE, Scopus, and Web of Science for studies published until February 26, 2026, including adult participants assessed for OCD symptoms and at least one phase of the sexual response cycle or genital pain. Data extracted included study design, participant characteristics, assessment tools, and the sexual functioning domains evaluated. Quality assessment was performed using the QUIPS Tool. RESULTS: We included 13 articles. OCD symptoms are associated with sexual desire, arousal, and female orgasm dysfunctions, likely due to the cognitive, emotional, and behavioral rigidity associated with the disorder. These interferences can impact both individual well-being and relationship satisfaction. No consistent evidence emerged regarding the association between OCD symptoms and male orgasmic function or genital pain; therefore, the currently available findings do not allow firm conclusions to be drawn about these relationships. Given the limited number of specific studies on this topic, further research is needed to improve clinical understanding and inform tailored treatment approaches that address both psychopathology and sexual health. CONCLUSION: The results of this study emphasize the association of sexual dysfunctions and OCD symptoms and the importance of evaluating sexual functioning in these patients, with a focus on enhancing their quality of life.
