Phenotypic Characterization and Antimicrobial Resistance Patterns of Clinical Klebsiella pneumoniae Isolates from Al-Najaf Teaching Hospital

Klebsiella pneumoniae is a common pathogen associated with both community and hospital-acquired infections, both of which present unique challenges for clinicians. Klebsiella pneumoniae is the second leading cause of healthcare-associated Urinary Tract Infections (UTIs), with multidrug resistance worldwide. Objectives: The current study aimed to determine the antimicrobial susceptibility patterns of Klebsiella pneumoniae isolated from different clinical samples from Al-Najaf teaching hospital in Al-Najaf city, Iraq. Methods: A cross-sectional study was conducted over a six-month period (January to June 2024), focused on Al-Najaf teaching hospital. A total of 84 clinical isolates Klebsiella pneumoniae were obtained from various clinical specimens. Antimicrobial susceptibility testing was conducted using minimum inhibitory concentration (MIC) methods using the VITEK-2 automated system according to standard guidelines. Results: Females revealed a slightly higher infection rate (60.7%) than males (39.3%). Urine were the most common source of isolation (40.5%), followed by sputum (28.6%), high vaginal swab (HVS) (9.5%), wound (7.1%), blood samples (4.8%) and other specimens included ear swabs, pharyngeal swabs, tracheal swab, throat swab and semen represented 9.5%. A high resistance levels were observed against ampicillin (97.6%) and piperacillin (88.1%), followed by cefotaxime (81%), amoxicillin/clavulanic acid (78.6%), ceftazidime (77.4%), Ceftriaxone (73.8%), Cefepime (63%) and low resistant isolates to imipenem and meropenem (11.9%), Colistin showed the highest sensitivity rate (100%). Conclusion: The study showed high prevalence of multidrug-resistant Klebsiella pneumoniae from various clinical specimens in Al-Najaf teaching hospital. Colistin was most active antimicrobial drug against K. pneumoniae isolates. As a result, continuous screening, surveillance and effective antibiotic stewardship are needed in Al-Najaf hospitals to limit the further spread of resistant isolates.
