Acute iliofemoral deep vein thrombosis treatment in the setting of an infrarenal inferior vena cava aneurysm

A 35-year-old man presented with a one-week history of progressive left lower-extremity pain and extensive swelling extending from the groin to the ankle, with a history of intermittent left leg heaviness since adolescence. Ultrasonography demonstrated extensive acute deep vein thrombosis (DVT), and computed tomography venography confirmed thrombus extending to the origin of the left common iliac vein (LCIV). Imaging revealed several anatomic abnormalities: chronic May–Thurner syndrome with congenital or secondary hypoplasia of the LCIV (A), aneurysmal dilatation of the infrarenal inferior vena cava (IVC) (B), and IVC scarring with partial fenestrations within the aneurysmal segment.
