Multifunctional exosome-infused antimicrobial mouthwash for oral ulcer treatment

Oral ulcers are common and recurrent, yet existing mouthwashes face limitations due to single-target effects, compositional complexity, or immunogenicity risks. Here, we develop a multifunctional composite mouthwash (E-C-MW) by integrating mesenchymal stem cell-derived exosomes (Exo) with cetylpyridinium chloride (CPC). Unlike growth factors or herbal mixtures, Exo offer intrinsic multi-target regulatory capacity within a naturally derived, providing a more biomimetic and mechanistically distinct approach to tissue repair. The rinse formulation of E-C-MW offers distinct advantages for oral ulcer management. E-C-MW is easy to use with high patient compliance and comfort, particularly given the significant pain associated with ulcer flare-ups. Moreover, its liquid nature allows superior coverage of posterior or anatomically hidden ulcers that are difficult to reach by other topical dosage forms. Within this convenient and comfortable rinse, E-C-MW achieves broad-spectrum antimicrobial action while actively promoting healing. In a rat oral ulcer model, E-C-MW significantly accelerates healing, outperforming common commercial mouthwashes and single-component treatments. By establishing Exo as a viable bioactive platform in daily oral care, this study not only provides a promising strategy for recurrent oral ulcers but also opens new directions for developing multifunctional therapeutic mouthwashes for oral mucosal diseases.
