The gut microbiota plays essential roles in maintaining homeostasis, acting in metabolism, immunity, and communication between the gut and the nervous system. Its composition can be influenced by several factors, including physical exercise. This study analyzed the effects of physical exercise on the gut microbiota of healthy physically active individuals, including athletes and recreational exercisers. A systematized literature review was conducted following PRISMA guidelines. Searches were performed in PubMed, OpenAIRE, Cochrane Library, and LILACS databases, covering open-access articles published between 2015 and 2025 involving healthy adults aged 18 to 59 years. Studies investigating the relationship between physical exercise and gut microbiota composition were selected, and data were organized and assessed for methodological quality.Of the 19,812 studies identified, 10 met the eligibility criteria. The findings demonstrated an association between physical exercise and changes in gut microbiota composition and diversity, with variations related to physical activity level, training intensity, and individual characteristics. Moderate-intensity exercise was associated with increased bacterial diversity and enrichment of potentially beneficial microorganisms, whereas intense and prolonged exercise was linked to changes in intestinal permeability, inflammatory responses, and microbial imbalance.In conclusion, physical exercise exerts a modulatory effect on the gut microbiota, with potential implications for metabolic and immune health as well as sports performance. These findings highlight the need for further studies to better understand the mechanisms underlying this interaction.
