Clinical Translation of Nanocarrier-based Drug Delivery Systems for the Treatment of Fatty Liver Disease

Fatty Liver Diseases (FLDs), including basic steatosis to Non-Alcoholic Steatohepatitis (NASH), are an increasing global health issue. Current therapeutic options for FLD are limited, highlighting the need for innovative approaches. Nanomedicines are revolutionising healthcare, with vaccines such as those from Pfizer/BioNTech and Moderna COVID-19 delivering billions of doses safely. This chapter highlights recent advances in nano-medicines for developing diagnostic and therapeutic tools for fatty liver disease without alcohol use, a growing global health challenge, paving the way for precision medicines. Nano carrier-based drug delivery systems (NDDS) offer a promising avenue for enhancing drug delivery to the liver, improving therapeutic efficacy, and minimising side effects. This chapter explores the potential of NDDS in FLD management, focusing on recent advancements in nanomaterial design, targeting strategies, and clinical translation, and discusses the challenges and opportunities associated with the development and clinical application of NDDS for FLD, including safety considerations, regulatory hurdles, and the need for well-designed clinical trials. Despite these challenges, NDDS has significant potential to improve the treatment of FLD and patient outcomes. The clinical trials investigating the efficacy of NDDS in FLD have shown promising results. Additionally, silibininloaded nanoparticles enhance mucosal penetration and oral absorption compared with conventional silibinin. Nanoparticulate approaches have shown promise in overcoming lipid metabolism disorders in preclinical trials, indicating potential effectiveness in humans.
