Impact of Osteopathic Manipulative Technique and Circuit Interval Training on Sympathetic Hyperactivity in Pcos Women

Background: Polycystic Ovary Syndrome (PCOS) is a multifactorial endocrine disorder with increased prevalence among women of reproductive age, especially among Indian adolescents. Apart from its reproductive consequences, PCOS has strong links to insulin resistance, dyslipidemia, and autonomic dysfunction, particularly increased sympathetic nervous system (SNS) activity, that add to its metabolic and cardiovascular load. Objective: To assess the effect of a combined intervention—Osteopathic Manipulative Treatment (OMT) and Circuit Interval Training (CIT)—on sympathetic hyperactivity and related metabolic, hormonal, and quality of life outcomes in women with PCOS. Methods: A longitudinal interventional trial was performed on 30 women in the age group of 25–40 years with PCOS, diagnosed according to Rotterdam criteria. The participants received weekly OMT sessions for Chapman's reflex points and standardized CIT protocols for 9–11 months. Outcomes were changes in COMPASS-31 (autonomic function), QOL scores, BMI, fasting glucose, insulin, HOMA-IR, serum testosterone, and menstrual regularity. Results: After intervention, remarkable improvement was noted in COMPASS-31 scores (−9.86), QOL scores (+11.88), BMI (−1.5 kg/m²), waist circumference (−4.5 cm), HOMA-IR (−1.13), and testosterone (−12.3 ng/dL). Menstrual regularity and ovulation rates significantly improved, and dermatologic features of hyperandrogenism decreased. The combined therapy successfully prevented autonomic dysregulation and systemic metabolic load. Conclusion: Combining OMT and CIT provides a new, multi-faceted therapeutic approach for PCOS, enhancing autonomic tone, endocrine equilibrium, and well-being. This drug-free therapy has substantial clinical promise and deserves further investigation through randomized controlled trials.
