1631-P: Unidos en Salud: Using a Mobile Unit for Implementing Diabetes Screening in Rural Latino Communities

Introduction and Objective: Despite facing high burden of type 2 diabetes, rural Latino communities have limited access to preventive screening services. Working with Latino community members, we co-designed Unidos en Salud (“United in Health”), a community-based mobile unit that provides diabetes screening and referral services. The objective of this study was to determine the reach and preliminary effects of a diabetes screening mobile unit among rural Latino communities. Methods: The mobile units were rolled out in two rural Indiana counties and were staffed by trained bilingual community health workers. The mobile unit offered random capillary glucose tests and HbA1c assessments for free. For users with elevated values (capillary glucose ≥100 mg/dL or HbA1c ≥5.6%), brief preventive counseling, a referral to accessible healthcare services, and a 3-month follow up visit were also offered. Baseline and follow-up mean glucose and HbA1c values were compared for these participants using Wilcoxon’s signed-rank test. Results: Across seven months, 29 screening events were held, reaching 186 individuals (mean age 43±12 years; 53% women). Most users were Latino adults (94%) and did not have health insurance (65%). Mean capillary glucose was 134.9 mg/dL and mean HbA1c was 6.1%. Seventy one percent of users were referred to additional clinical services due to elevated results: 41% (n=76) due to capillary glucose or HbA1c levels falling in the prediabetes range, and 16% (n=30) due to values falling in the diabetes range. The follow up visits are ongoing and 16 users have completed this visit so far; their test results show no significant improvement in capillary glucose (182 vs. 144 mg/dL, p=0.25), or HbA1c (6.6% vs. 6.4%, p=0.97) from baseline. Conclusion: Preliminary results suggest that a screening mobile unit is a promising model for detecting individuals at high diabetes risk in rural Latino communities.
