Introduction and Objective: Automated insulin delivery (AID) systems have demonstrated clinical benefits in Latin America (LATAM)1. While AID therapy benefits are well established, evidence of sustained real-world performance is critical to support broader adoption and long-term regional implementation. This analysis reports 3-year glycemic outcomes among Latin American users of the MiniMed™ 780G system (MM780G), providing evidence of long-term effectiveness and feasibility of AID in a diverse population. Methods: Carelink™ data (Jan 2020 to Oct 2025) of consenting MM780G users aged ≥7 years from five LATAM countries (Argentina, Brazil, Chile, Colombia and Mexico) with self-reported T1D were analyzed. Glycemic outcomes and adoption of the recommended optimal settings (ROS: glucose target 100 mg/dL and active insulin time 2h, for ≥95% of the time) were explored over three years. Results: Overall, T1D users (N=307) consistently met all consensus-recommended targets (Figure 1). All glycemic metrics remained stable throughout the analysis period. Time in tight range (TITR, 70-140mg/dL) was sustained above 50% across the 36 months examined. Adoption of ROS increased from 19.0% in month 1 to 28.7% in month 36. Conclusion: These findings provide robust RWE of the long-term effectiveness and feasibility of the MM780G system in LATAM. This sustained glycemic performance highlights the real potential for AID to improve diabetes management, supporting broader adoption in the region. Disclosure M.
