2327-P: Impact of Cystic Fibrosis–Related Diabetes on Sarcopenia Prevalence

Introduction and Objective: Malnutrition is a well-recognized complication of cystic fibrosis (CF), resulting from chronic malabsorption due to exocrine pancreatic insufficiency, reduced appetite, and increased energy expenditure, often leading to sarcopenia. Diabetes is an additional risk factor for muscle loss; however, data on sarcopenia prevalence in CF and the specific impact of cystic fibrosis-related diabetes (CFRD) remain limited. This study aimed to assess the prevalence of sarcopenia and to identify associated risk factors in people with CFRD. Methods: Eighty-nine adults with CFRD were enrolled in this observational study. Clinical history, anthropometric parameters, and laboratory data were collected. Sarcopenia was assessed according to the European Working Group on Sarcopenia in Older People 2 (EWGSOP2) criteria using bioelectrical impedance analysis, handgrip strength, and walking performance tests. Results: The prevalence of sarcopenia in our population was 12.5%. Participants with sarcopenia showed significantly poorer glycemic control compared with non-sarcopenic participants, as reflected by higher HbA1c levels (7.3±2.8% vs. 6.6±1.0%, p=0.05) and fasting plasma glucose (141.4±47.2 vs. 110.5±31.8 mg/dL, p<0.01). Sarcopenic individuals also had significantly lower body mass index (19.1±2.7 vs. 23.6±3.3 kg/m², p<0.01), higher fat mass (22.4±5.7 vs. 20.3±8.7%, p<0.01), and lower muscle mass (77.5±5.7 vs. 79.3±9.8%, p=0.01). Conclusion: Preliminary data suggest that poor glycemic control in CFRD may represent an additional risk factor for sarcopenia in people with CF, a population already predisposed to muscle loss. Optimizing glycemic control may therefore be crucial to prevent sarcopenia and its detrimental effects on respiratory function and survival.
