1979-P: Are We Catching T2DM Early Enough? Retrospective Study of Medical Student Screening Program for Prediabetes/T2DM in an Underserved Population Demonstrates Gaps in Diagnosis in South Carolina

Introduction and Objective: One in seven South Carolinians have type 2 diabetes mellitus (T2DM), and patients who have a low income or are uninsured are more likely to experience significant complications of diabetes. Medical Students Making Change (MSMC), a student-led organization at the University of South Carolina School of Medicine, aims to improve glycemic control in these patient populations through earlier diagnosis via universal screening at an underserved clinic. Methods: Medical student volunteers screen all clinic patients collecting their HbA1c and blood glucose levels at a local clinic that primarily serves an underserved population whose patients are universally below the poverty line. Patients are diagnosed with prediabetes or T2DM based on HbA1c by the clinic medical staff and endocrinologist. Results: All patients screened are South Carolina residents that were at or below 200% of the Federal Poverty Level with no health coverage. Of patients screened (n=868), 57.3% of patients have T2DM (44.7%; n=388) or prediabetes (12.6%; n=109). Mean HbA1c of clinic patients was 6.5%, and mean blood glucose was 170 mg/dl. Most importantly, the screening process identified a significant cohort of previously undiagnosed patients (19.1%; n=95), whose mean HbA1c was 7.6% and mean blood glucose was 112 mg/dl. Conclusion: The current results demonstrate the importance of more extensive screening in high-risk populations for prediabetes and T2DM.
