Colistin, a last-resort antibiotic against multidrug-resistant pathogens, has faced growing resistance threats since the discovery of plasmid-mediated mcr genes in 2015, enabling horizontal transfer of resistance across species and ecological niches. This systematic review and meta-analysis aims to synthesize the global evidence on the prevalence and molecular epidemiology of mcr genes and chromosomal mutations associated with colistin resistance in Gram-negative isolates across human, animal, food, and environmental sources worldwide, to estimate pooled prevalence where data permit, and to characterize the phylogenetic relationships and clonal dissemination patterns of resistant strains across regions.
