Effect of a smartphone application on dispatcher-assisted CPR and AED response in simulated out-of-hospital cardiac arrest

Background: Early bystander cardiopulmonary resuscitation (CPR) and rapid defibrillation are critical determinants of survival in out-of-hospital cardiac arrest (OHCA). Objective: This study evaluated the effectiveness of a smartphone application designed to support dispatcher-assisted CPR and bystander defibrillation in simulated OHCA scenarios.Methods: This simulation-based randomized study enrolled emergency medical dispatchers and trained lay responders who were randomly assigned to either an intervention group using the smartphone application or a control group following standard procedures. The primary outcomes were time to initiation of bystander CPR and time to first automated external defibrillator (AED) shock delivery. Secondary outcomes included high-quality CPR (HQ-CPR) performance and chest compression depth.Results: Fifty emergency medical dispatchers participated (intervention n = 25; control n = 25). Dispatchers in the intervention group demonstrated a significantly shorter time to CPR initiation compared with the control group. Among 190 lay responders (intervention n = 95; control n = 95), participants using the smartphone application showed significantly faster detection time and overall response time. However, time to CPR initiation was longer in the intervention group compared with the control group. No significant differences were observed in AED shock delivery time, HQ-CPR performance, or chest compression depth between groups.Conclusion: In simulated OHCA scenarios, the smartphone application improved early detection and overall response time but demonstrated differential effects on CPR initiation. While dispatchers demonstrated faster CPR initiation, lay responders experienced a delay. No significant differences were observed in time to AED shock delivery. These findings suggest that mobile technology may enhance early response processes, while highlighting the need to optimize user interaction to minimize delays in initiating CPR.
