A targeted snapshot multispectral sensor for enhanced surgical vision (Conference Presentation)

The clinical standard for surgical vision (e.g. endoscopy, laparoscopy) uses cameras with red, green and blue filters to mimic human vision, but these RGB cameras often have poor contrast for early pathologies. Contrast-enhanced imaging modalities to supplement or replace RGB cameras often face barriers to adoption if they require specialist training, disrupt clinical workflow, impede real-time visibility or rely on multiple devices being used in parallel. We present a snapshot multispectral camera based on a targeted 9-band vis-NIR filter array (BioMSFA) integrated on a 4K CMOS image sensor. The BioMSFA wavebands (420-830nm) are optimised to enhance contrast of blood vessels, oxygenation, and surgical NIR-fluorescent dyes simultaneously on a single chip. The nine filters of the BioMSFA are arranged to mimic the RGB-Bayer layout, to retain backward-compatibility with existing RGB image processing systems, to minimise adoption barriers. Additionally, embedded within the BioMSFA are 16-band hyperspectral regions, which enable high resolution point spectral measurements to be taken from regions of interest within the image, in parallel to the multispectral imaging. A BioMSFA prototype was manufactured in-house via a scalable method for cost-effective mass production. The prototype’s spectral accuracy and spatial resolution were assessed in reference to calibrated colour charts and resolution targets. Blood oxygenation mapping was quantified using a tissue mimicking phantom filled with flowing blood, whose oxygenation was varied from 0-100% in ~10% intervals. In vivo video-rate perfusion mapping was performed on a healthy volunteer undergoing cuff occlusion, and fluorescence imaging was demonstrated using ICG mimicking phantoms. Results from the prototype testing indicate that the BioMSFA design has the potential to augment surgical vision with diagnostically relevant blood oximetry and perfusion information, while rapid interleaved illumination allows the sensor to be used for pseudo-simultaneous enhanced contrast and fluorescence imaging without the need for multiple sensors and image co-registration.
