FAR-POLYP-SEG: A Prospective Colonoscopy Image Dataset for Colorectal Polyp Segmentation with Patient-Level Metadata FAR-POLYP-SEG is a prospectively collected colonoscopy dataset acquired at Farhikhtegan Hospital, Islamic Azad University, Tehran, Iran (February–December 2025), during routine clinical procedures using standardized Olympus 190-series equipment. All frames are de-identified. Contents: 8,181 RGB frames from 455 patients: 432 polyp-positive frames with expert-verified pixel-level binary masks and annotation overlays 7,749 normal colonic mucosa frames (frame-level polyp prevalence ≈ 5.3%) patient_summary.csv with per-patient metadata: age, sex, colonoscopy indication, Boston Bowel Preparation Scale (BBPS) score, procedure duration, and frame counts Layout: One folder per patient (Patient1/ … Patient455/), each containing Healthy/, Polyp/, Annotated/, and BinaryMask/ subfolders. Polyp/Annotated/BinaryMask files share filenames within each patient. Ethics: IRB approval IR.IAU.FARHIKTEGANH.REC.1403.004 (Farhikhtegan Hospital, approved 2024-07-10).
