Signs of clinical deterioration in adults with hematologic malignancies in hospital inpatient units

This protocol outlines a scoping review aimed at mapping the evidence on signs of clinical deterioration in adults with hematological malignancies hospitalized in general inpatient units. Hematological neoplasms present complex and unpredictable clinical trajectories, involving aggressive treatments that generate serious complications across multiple body systems. Although early recognition of clinical deterioration is critical to reducing in-hospital morbidity and mortality, widely used Early Warning Systems present relevant limitations in this population, including low predictive accuracy and models not calibrated for onco-hematological specificities such as febrile neutropenia. This review will include primary studies of quantitative, qualitative, or mixed-methods designs, reviews, and gray literature. Searches will be conducted across eleven electronic databases without language or time restrictions. Study selection will be performed independently by two blinded reviewers, with disagreements resolved by a third. Data will be extracted using a structured checklist and presented descriptively in tables, charts, and figures. Methodological quality will be assessed using JBI critical appraisal tools appropriate to each study design. The findings are expected to broaden the clinical parameters used by nursing teams in monitoring this population, support the development of more sensitive early warning tools, and promote timely interventions capable of altering the clinical course of these patients.
