Neurological, neurophysiological, neuroanatomical, neuropathological, and neurodevelopmental consequences of childhood maltreatment: a scoping review

Background: Childhood maltreatment is associated with long-term mental, developmental, and biological consequences. However, the neurological and neurobiological pathways linking maltreatment to altered brain development remain heterogeneous and incompletely mapped. Objective: To map the available evidence on neurological, neurophysiological, neuroanatomical, neuropathological, and neurodevelopmental alterations associated with childhood maltreatment in children and adolescents. PCC: Population: children and adolescents exposed to childhood maltreatment. Concept: neurological, neurophysiological, neuroanatomical, neuropathological, and neurodevelopmental alterations. Context: clinical, neuroimaging, neurophysiological, neuropathological, and translational human studies. Methods: This scoping review will be conducted according to PRISMA-ScR and registered on the Open Science Framework. Searches will be performed in PubMed/MEDLINE, Embase, Cochrane Library, Web of Science, and Google Scholar. Records will be imported into Rayyan, duplicates removed, and titles/abstracts screened independently by paired reviewers. Disagreements will be resolved by consensus or by a senior reviewer. Eligibility: Human studies including children or adolescents exposed to childhood maltreatment, abuse, neglect, early-life adversity, or adverse childhood experiences, reporting at least one neurological, neuroimaging, neurophysiological, neuropathological, neuroinflammatory, neuroendocrine, or neurodevelopmental outcome. Exclusions: Purely psychiatric or psychosocial studies without neurological/neurobiological outcomes; adult-only studies without pediatric data; animal studies; narrative reviews; editorials; opinion pieces. Data extraction: author, year, country, design, population, age, maltreatment type, assessment method, neurological domain, tool used, brain region/biomarker/outcome, main findings, limitations. Synthesis: descriptive numerical summary, evidence map by domain, tables of included studies, and identification of research gaps. Meta-analysis will not be planned initially but may be considered if homogeneous outcomes are identified.
