The Impact of Vaccine Administration Training on Motivational Interviewing and Procedural Skills in Medical Education

Children rely on caregivers being up-to-date with their own vaccinations to provide local herd immunity, a strategy known as cocooning in infection prevention. Since 2011, the Friends and Family Vaccine Program at Riley Hospital for Children has offered influenza and Tdap vaccines to close contacts of hospitalized pediatric patients. As part of their pediatric rotation, third-year medical students spend a half day providing vaccine counseling and administering vaccines t[HB1.1]hroughout the hospital. Since incorporating medical students into the program, inpatient vaccine administrations increased by 123% during the 2023–24 viral season and by 85% during the 2024–25 season. Medical students administered 86% and 75% of all inpatient vaccines during these seasons, respectively, highlighting their substantial contribution to the program’s success. The objective of this study is to evaluate the utility of the vaccine program as part of the medical school curriculum in improving students’ vaccine knowledge, comfort with motivational interviewing and counseling, and experience performing procedures. We anticipated that many students would have limited prior exposure to these skills and that participation would provide early, broadly applicable clinical experience. Students completed pre- and post-session surveys assessing prior experience and comfort with vaccine counseling, procedures, and motivational interviewing. During the session, students received instruction on vaccine knowledge, motivational interviewing techniques, the consent process, and vaccine administration. They then practiced these skills by counseling patients’ family members and administering vaccines under supervision. Data collection is ongoing during the current viral season. Preliminary results suggest that many students had little prior procedural experience and reported increased comfort with counseling, interviewing, and vaccine administration after the session. Overall, integrating the vaccine program into the third-year medical curriculum appears beneficial for early skill development. Future plans for the program include expanding the program to first- and second-year medical students.
