Rethinking Anatomy in Hernia Repair: Paediatric Percutaneous Experience and Adult Implications

What if the structure we remove is part of the repair? This chapter invites the reader to rethink inguinal anatomy and to question “standard” steps that often violate the canal’s architecture. Through four lenses – history, anatomy, tailored paediatric technique, and outcomes – we argue that closing the internal ring under vision while leaving the sac undisturbed offers a simple, anatomy-preserving solution for indirect hernias. We present FLAIR (fenestrated laparoscopic-assisted internal ring-rrhaphy) as a reproducible expression of this approach: a percutaneous closure that respects key structures and avoids posterior-wall disruption, with reassuring results and minimal morbidity. The aim is not to replace judgement but to widen it – encouraging surgeons to look again at the sac, the ring, and the least-disruptive repair. We conclude by outlining how these principles could be prospectively applied in adults with primary indirect hernias.
