Background Multiple long-term conditions (MLTC) is an increasing public health issue. MLTC is linked with poorer health, reduced mobility, lower quality of life, premature mortality, and higher use of health care. Evidence suggests that remission -the resolution or cessation of the disease- in various individual long-term conditions (LTCs) is associated with improved health outcomes. However, there is a lack of research on remission in the context of MLTC. Methods We conducted an observational cohort study using Clinical Practice Research Datalink (CPRD) data to identify remission of individual LTCs in adults with MLTCs. Descriptive statistics were used to report the frequency and patterns of remission of individual LTCs and to quantify remission by age, sex, ethnicity, and socioeconomic status. Logistic regression models and Cox proportional hazards models were fitted to estimate associations of remission with baseline characteristics (age,sex, ethnicity, socioeconomic status) and mortality, respectively. Results Of the 4,745,329 adults in our cohort, 1,475,550 (31%) achieved remission of ≥1 of our 11 LTCs over a median follow-up of 8.4 (3.7-14.5) years. Remission of LTCs in 198066 (13.4%) people were identified based on remission codes. Most people achieved remission in one condition only [n=1,242,188 (84.2%)]. The LTCs that achieved remission more commonly were anaemia, asthma, and type 2 diabetes. Men were less likely to achieve remission than women [adjusted OR (95% CI): 0.74(0.73-0.74]. Black and Asian ethnicities were more likely to achieve remission [aOR: 1.13(1.2-1.14); aOR: 1.33(1.32-1.33)]. Remission was associated with decreased risk of mortality (aHR:0.78(0.78-0.78). Conclusion The study identified that remission is possible in the context of MLTC and highlighted conditions and groups of people that may be targeted by interventions aimed at achieving remission and improving outcomes of MLTC. Our findings further indicate that remission is recorded for selected LTCs in routine data and can be incorporated into routine management and monitoring of conditions.
