An Initiative between the Diabetes Integrated care team and Portiuncula university hospital ( Diabetes team was undertaken with aim to reduce wait list times for people with Type 2 Diabetes in PUH. A Validation process between integrated care team, Portiuncula University Hospital (PUH) Diabetes team was completed highlighting patients on the Acute Diabetes waiting list who fulfilled the criteria of the Integrated Diabetes hub Appointments were then scheduled for these people in local Primary Care Centres close to their home by the integrated care diabetes team.
