The Prognostic Impact of Type 2 Diabetes on Renal Cell Carcinoma Stage, Recurrence, and Mortality: A Nationwide Danish Cohort Study (2004-2024)

Renal cell carcinoma (RCC) is a common urological cancer, with approximately 1,100 new cases in Denmark every year (https://www.cancer.dk/nyrekraeft/fakta/statistik/). Type 2 diabetes (T2D) affects an estimated 7.1% of the adult Danish population (approximately 350,000 individuals), and its prevalence is expected to increase (https://www.diabetestal.nu/). Cancer has furthermore become the leading cause of death among individuals with T2D (DOI: 10.1016/j.lanepe.2024.100909). Existing evidence suggests that T2D is associated with an increased risk of developing RCC, as well as higher recurrence rates and lower cancer-specific survival (DOI: 10.2337/dc22-0961, DOI: 10.1002/ijc.34793). This nationwide, population‑based cohort study includes all Danish adults diagnosed with incident RCC (ICD-10 C64.9) between 1 January 2004 and 30 September 2022, recorded in the Danish Cancer Register (DOI: 10.1177/1403494810393562). Pre-existing T2D will be identified using a validated register-based algorithm (DOI: 10.2147/CLEP.S407019). Individuals with RCC will be followed from the date of RCC diagnosis until the occurrence of the relevant outcome, death, emigration, or end of follow-up, whichever occurs first. Follow-up for recurrence and RCC-specific mortality will continue through 31 December 2022, while follow-up for overall mortality will continue through 31 December 2024. The aim of the study is to examine whether pre-existing T2D is associated with: 1. More advanced stage at diagnosis; 2. higher cancer recurrence risk; 3. poorer RCC-specific and overall survival.
