192 Alteration of Neuroinflammation and Intracranial Microbiome With Antibiotic Impregnated Catheters

INTRODUCTION: Ventricular catheter obstruction is a leading cause of shunt failure. While different catheter materials are used clinically, their impact on obstruction mechanisms remains unclear. the recent discovery of intracranial 16S rRNA introduces a potential microbiome-related modulator of neuroinflammation. METHODS: Twenty-nine female C57BL/6 mice were divided into four groups: control (n=6), sham surgery (n=6), plain silicone catheter (PS, n=6), and antibiotic impregnated silicone catheter (AIS, n=6). the rest five were used for MRI. Catheters were implanted in both the parietal cortex and peritoneal cavity. MRI was performed at 1, 4, 8, and 16 weeks on five mice on an 11.7 Tesla Bruker Biospec MRI system, using T2-weighted, FLAIR, and SWI sequences following ferumoxytol injection. Brain and cecum samples were collected at 7 and 28 days for microbiome analysis. Host reads were removed using Kraken2 with a GRCm39 mouse genome database. Reads were processed via DADA2 and QIIME 2; diversity metrics and differential abundance (LEfSe) were assessed in R. PICRUSt2 was used for functional inference. RESULTS: Peri-implantation edema was similar across implant groups and resolved by week 4. MRI revealed reduced macrophage activity in the AIS group after week 4. 16S profiling showed a treatment-specific shift: AIS mice had increased anti-inflammatory taxa (e.g., Lachnospiraceae, Akkermansia), while PS and sham groups were enriched in pro-inflammatory taxa (e.g., Gammaproteobacteria, Flavobacteriia). Functional prediction suggested decreased LPS and increased SCFA pathways in the AIS group, aligning with imaging data. CONCLUSIONS: Catheter material influences neuroinflammation and intracranial microbiota. AIS implantation yielded reduced inflammatory activity and a microbiome profile consistent with immune tolerance. These findings suggest a microbiota-mediated mechanism of implant compatibility.
