Highlights The literature review is dedicated to elucidating the origins of atrial fibrillation research and the key mechanisms underlying its development. The review discusses the contributions of prominent figures in the field of medicine to the advancement of atrial fibrillation surgery. The manuscript not only outlines the primary procedures performed in open atrial fibrillation surgery but also elaborates on the mechanisms of action of various energy sources. Abstract Atrial fibrillation, as the most prevalent sustained cardiac arrhythmia, remains a significant challenge in cardiac surgery. Open surgical approaches, originating with the development of the Cox-Maze procedure in the 1980s, aim to eliminate ectopic foci by creating controlled ablation lines. Modern open surgery for atrial fibrillation has evolved through the integration of energy-based ablation modalities, such as radiofrequency and cryoablation, which enable the formation of transmural lesions with reduced invasiveness. The purpose of this review article is to provide an overview of the historical development of surgical interventions for atrial fibrillation.
