Concurrent ocular melanocytosis and peribulbar dermoid cyst

A 4-year-old boy presented with a dark-gray discoloration of the sclera of the right eye, suggestive of ocular melanocytosis (OM) [Fig. 1a.]. In the left eye, a painless mass, present since birth, was at the left lateral canthus, which was covered with skin and hair follicles, suggestive of a dermoid cyst [Fig. 1a]. Fig. 1b shows the lesion in the left eye partly connected to the conjunctiva and partly to the lid margin and skin at the lateral canthus. Ocular melanocytosis occurs mainly in Asians and is due to an accumulation of fusiform dendritic melanocytes.[1] The dermoid was managed by an excision biopsy along with direct lateral canthal reconstruction and amniotic membrane grafting as there was bare conjunctiva after mass excision. The histopathology report confirmed the presence of a dermoid cyst. The concurrent presentation of ocular melanocytosis and peribulbar dermoid cyst has not been reported in previous literature and is a rare and novel finding.Figure 1: (a) Demonstration of the presence of ocular melanocytosis (OM) in the right eye without involvement of the skin and a lateral canthal dermoid cyst on the left side.
