Esettanulmányunkban egy 61 éves férfi kivizsgálását mutatjuk be, aki négy éve észlelhető fokozatos vesefunkció-romlás és ismeretlen ideje fennálló enyhe hypercalcaemia, hypophosphataemia és leukocytosis társulása miatt jelentkezett szakrendelésünkön. Kórelőzményében hypertonia, hyperlipidaemia, lumboischialgia, Hashimoto-thyreoiditis szerepelnek. 2019-ben a GFR 87 ml/min volt, 2024-ben a GFR 57 ml/min lett, jelentősebb csökkenést az utolsó másfél évben mutatott, ebben egy éven át tartó folyamatos NSAID-szedés is szerepet játszott. Vizeletvizsgálat során kalcium-karbonát kristály ürítés látszott egyéb eltérés nélkül. PTH a normáltartomány felső határán volt. Az NSAID leállítását javasoltuk. Dohányzás, hypercalcaemia és leukocytosis társulása miatt először tüdőtumor irányában mellkas-CT-vizsgálat történt, ez daganatot nem igazolt és sarcoidosis jeleit sem mutatta. Ezt követően osteoporosis, D-vitamin-hiány okozta szekunder hypercalcaemia merült fel, DEXA-vizsgálat nem jelzett érdemi csontritkulást, D-vitamin-pótlás pedig jelentős kalciumszint-emelkedést váltott ki. Nem szupprimált PTH-szinttel járó primer hyperparathyreosis merült fel, emiatt endokrinológiai szakvizsgálat történt. Familiáris hypocalciuriás hypercalcaemiát a gyűjtött vizeletben észlelt fokozott kalciumürítés kizárta, ezért PTH-dependens hypercalcaemiát állapítottak meg, amely egyelőre kezelést nem igényel. | We present the evaluation of a 61-year-old male patient who was referred to our outpatient clinic due to progressive renal function decline observed over the past four years, along with mild hypercalcemia, hypophosphatemia, and leukocytosis of unknown duration. His medical history includes hypertension, hyperlipidemia, lumboischialgia, and Hashimoto’s thyroiditis. In 2019, his GFR was 87 ml/min, which declined to 57 ml/min by 2024, with a more significant decrease in the last one and a half years, partially attributed to continuous NSAID use over a one-year period. Urine analysis revealed the presence of calcium carbonate crystals without other abnormalities. Parathyroid hormone (PTH) levels were at the upper limit of the normal range. NSAID discontinuation was recommended. Given the association of smoking, hypercalcemia, and leukocytosis, a chest CT scan was performed to rule out lung cancer, which did not confirm malignancy and showed no signs of sarcoidosis. Subsequently, secondary hypercalcemia due to osteoporosis and vitamin D deficiency was considered. However, a DEXA scan did not indicate significant osteoporosis, and vitamin D supplementation led to a marked increase in serum calcium levels. Primary hyperparathyroidism with non-suppressed PTH levels was suspected, prompting an endocrinological consultation. Familial hypocalciuric hypercalcemia was excluded based on increased urinary calcium excretion in the collected urine sample, confirming PTH-dependent hypercalcemia, which currently does not require treatment.
