Application of stem cells in selected rheumatological diseases

This review article examines the use of mesenchymal stem cells (MSCs) in the treatment of Sjögren's syndrome (SS), rheumatoid arthritis (RA), osteoarthritis (OA), and their potential in joint regeneration. Clinical analyses and animal studies have shown that umbilical cord-derived MSCs (UCMSCs) in patients with SS help alleviate disease symptoms, reduce pathological changes in the submandibular glands, such as local inflammation and fibrosis, and decrease the production of pro-inflammatory factors, including tumor necrosis factor-alpha (TNF-α) and interferon gamma (IFN-γ). Treatment with adipose-derived MSCs (ADMSCs) in patients with primary SS improves secretion from salivary and lacrimal glands, reduces disease activity indices, and lowers levels of immunoglobulin (Ig) G, IgM, complement factors 3 and 4, and erythrocyte sedimentation rate. In RA, MSCs modulate local immune responses, adjusting their function based on the severity of inflammation. Intra-articular injections of UCMSCs lead to significant, sustained improvements in knee mobility, pain relief, and quality of life for several weeks. Furthermore, MSCs have the ability to produce matrix metalloproteinase inhibitors, which may prevent cartilage damage.
