180 Training in Diagnostic and Therapeutic UGI Endoscopy Amongst Surgical Trainees in the United Kingdom and Northern Ireland: Results of a National Survey

Abstract Aim Endoscopy training in the United Kingdom and Northern Ireland is well structured and regulated through the Joint Advisor Group. Yet, there is a sense of disparity between the opportunities for surgical trainees vs gastroenterology trainees. We conducted a national survey to understand the realities amongst trainee surgeons. Method A 38-question survey was disseminated through AUGIS mailing group and Roux WhatsApp group and the 61 initial replies covering 19 deaneries were analysed. Results The majority felt that competence in endoscopy is essential for benign UGI and OG surgeons but not HPB surgeons. 39% had no access to regular lists and 15% had one weekly list. 70% did not have endoscopy timetabled into rota, 8% attended immersion training and 62% performed fewer than 150 OGDs. The majority experienced conflicting on-call and operative commitments, and preferential allocations of gastroenterology trainees. 18% had JAG accreditation and 15% performed independent OGD lists. 71% were interested in training in therapeutic endoscopy (dilatations, stents and management of post-operative complications) but only 20% had training access. 38% were interested in ERCP and EUS training but only 8% performed ERCP and 3% performed EUS; 15% planned a fellowship. 54% felt gastroenterology trainees and/or nurse endoscopists were prioritised. 38% had support, whilst 18% were discouraged from training in advanced endoscopy by CS/AES/TPD and 8% by gastroenterologists. Conclusions There is strong willingness amongst trainee UGI surgeons to further their endoscopy training, and yet support is lacking and opportunities limited. We suggest tapping this talent pool will further benefit the national agenda.
