The importance of quality of life has become a focus in many patients as survival rates continue to rise with advancements in treatment. This is very important as it relates to fertility preservation in young women who have not yet completed their families. Radiotherapy is commonly included in the treatment of pelvic malignancies, leading to ovarian failure, decreased uterine volume, reduced distensibility due to myometrial fibrosis, vascular damage, and endometrial injury. Uterine transposition has emerged as a novel potential alternative for preserving fertility in these patients ( 9 , 10 ). Its primary indications include rectal cancer, pelvic sarcomas, anal canal cancers, as well as vaginal and vulvar cancers ( 11 ). This chapter provides a detailed overview of the surgical technique involved in uterine transposition, along with discussions on its key surgical considerations and outcomes. The procedure is a promising viable option for preserving both the uterus and gonadal function, leading to successful spontaneous pregnancies.
