Extract International guidelines currently recommend the use for at least 6 months of azithromycin for children with non-cystic fibrosis (CF) bronchiectasis in case of >1 hospitalized or ≥3 non-hospitalized respiratory exacerbations in the previous 12 months [1, 2]. Such long-term treatment halves the frequency of exacerbation [3, 4], and possibly reduces hospitalization [2]. Question is when to stop this treatment which significantly increases macrolide-resistant bacteria carriage in the upper airway [5] and whose maximal efficacy is noted between the 17th and 62th week following initiation [6].
