Abstract Purpose: As part of its goal to represent the National Cancer Institute (NCI) and promote its engagement with international partners, the NCI Center for Global Health (CGH) conducts an annual analysis of NCI’s international research portfolio, which includes NCI-funded extramural grants and NCI intramural projects with principal investigators (PIs) or collaborators at institutions outside of the U.S. This study examines recent NCI-funded cancer screening and prevention research in low- and middle-income countries (LMICs). Methods: NCI-funded extramural grants active in fiscal year 2024 (FY24) with any foreign collaborator were identified in National Institutes of Health’s (NIH) internal database, Query-View-Report. PI-led intramural research projects in FY24 with a foreign collaborator were reported through a data call fielded by NCI CGH. Grants were categorized by common scientific outline (CSO) code and cancer site according to Dimensions for NIH. Analysis was conducted in Microsoft Excel and Python. Results: NCI funded 1,097 extramural research grants and 271 intramural research projects with international collaborators in FY24. Of these grants, 348 (32%) had at least one collaborator in an LMIC, including 14 grants with principal investigators in LMICs. The most common area of research in the LMIC extramural portfolio was Early Detection, Diagnosis, and Prognosis (117 grants), which was the third most common research area in the overall international portfolio. Of the LMIC extramural portfolio, 77 grants were coded to Prevention, which was the fifth most common CSO code. Of the intramural international portfolio, 49 projects (18%) had at least one collaborator in an LMIC. Of these, Prevention and Early Detection, Diagnosis, and Prognosis had 6 projects coded to each area of research. This tied for the fourth most common CSO code. The analysis presents trends in geographic, income group, cancer site, and CSO code distribution of the identified grants and projects. Conclusion: NCI supports a broad and growing portfolio of research with international collaborators. While most of the research includes collaborators in high income countries, there is increasing collaboration with institutions in LMICs. Cancer screening research is well represented in this subset of the portfolio, but prevention research is underrepresented. This analysis aids our understanding of existing collaborations and identifies gaps in research funding. Citation Format: Tosca Le, Elise M. Garton, Kalina Duncan.
