• Uterine carcinosarcoma is rare, aggressive, and often misdiagnosed. • Early detection is crucial, especially in low-resource settings. Postmenopausal bleeding is a common presentation of gynecological malignancies, often associated with endometrial carcinoma. However, uterine carcinosarcoma is a rare and aggressive malignancy that can present similarly. This case report describes a 55-year-old woman with postmenopausal bleeding and hematometra, raising a high clinical suspicion of endometrial carcinoma, who was ultimately diagnosed with uterine carcinosarcoma on histopathologic examination. This case highlights the diagnostic challenges and the importance of histopathological evaluation in atypical presentations.
