Emerging Treatment Options for Relapsed/Refractory Chronic Lymphocytic Leukemia/Small Lymphocytic Lymphoma

For relapsed/refractory chronic lymphocytic leukemia (CLL), important considerations for selecting the patients’ next therapy include consideration of prior treatments, including the reason for discontinuation (ie, intolerance, progression, or completion of planned time-limited treatment). Covalent Brüton tyrosine kinase (BTK) inhibitors and venetoclax are backbone therapies in the era of targeted agents and may be sequenced. For patients at high-risk who have experienced disease progression during or shortly after both a covalent BTK inhibitor and a BCL2 inhibitor (ie, double-refractory CLL), the noncovalent BTK inhibitor pirtobrutinib and the CD-19–directed CAR T-cell therapy lisocabtagene maraleucel (liso-cel) are established as new standards of care. The therapeutic pipeline is expanding with novel agents that are being explored in clinical trials, including next-generation BTK protein degraders and T-cell–engaging bispecific antibodies. The paradigm of individualized, sequential therapy aims to maximize long-term disease control in patients with CLL.
