Evaluating physical activity levels in patients with migraine: insights from a tertiary headache center using the Exercise Vital Sign tool

Dear Editor, Physical activity (PA) is associated with lower frequency, intensity, and duration of migraine.1,2 The US Department of Health and Human Services recommends that adults do at least 150 minutes per week of moderate- to vigorous-intensity PA (MVPA).3 Exercise Vital Sign (EVS) is a validated screening tool to identify patients who are not meeting the PA guidelines.4,5 The EVS asks (1) “On average, how many days per week do you engage in moderate to strenuous exercise (like a brisk walk)?” and (2) “On average, how many minutes do you exercise per day at this level?”4 Although the EVS is used as a screening tool in large health care systems for different conditions, its use in a tertiary headache center has not been reported. The primary aim of the present study was to use the EVS to evaluate PA levels among new patients with a confirmed diagnosis of migraine according to the International Classification of Headache Disorders (ICHD)-3 criteria (https://ichd-3.org) at a tertiary headache center. Secondary aims focused on comparing PA levels between patients with episodic migraine and those with chronic migraine and on evaluating potential correlates. The main hypothesis was that the EVS would identify insufficient PA levels in patients with migraine, especially with chronic migraine. It was hypothesized that higher levels of depression and anxiety symptoms, increased body mass index (BMI), and more significant migraine-related disability would be associated with insufficient PA levels (<150 MVPA min/wk).
