Determination the frequency of drug-resistant causative pathogens and associated risk factors in Non-ICU HAP ("TR-HAP study")

Resistant microorganisms are more common as ventilator associated pneumonia agents, but have recently been detected in wards. No study has been found in the literature on the frequency of resistant pathogens, associated factors and mortality rates in Non-ICU HAPs in our country. We planned as the Turkish Thoracic Society “Respiratory System Infections Study Group”, to determine the resistant causative pathogens, associated risk factors and treatment success in non-ICU HAPs in Turkey, multicenter study. Patients were recorded in a common database from 22 centers that accepted to participate in the prospective, observational study. The first 6 months of the study, planned for one year, were analyzed and preliminary results are presented. A total of 307 patients, 106 (34.5%) of whom were female and mean age was 66.06±15.60 year were included in the study. Total of 65 out of 307 patients (21.1%) had multidrug-resistant (MDR) agents. The Non-ICU HAP patients with MDR agents were older and most of them were male. History of previous iv antibiotic use and, MDR agent isolation within the last 1 year were significantly higher in the group with MDR agents. Clinical recovery and survival rates at the end of the 6th week were also higher in the group without MDR agents (p=0.002, p=0.001, respectively). According to multivariate regression analysis, the probability of MDR isolation was 2.59 times higher in men than in women and 10.4 times higher in those with a history of MDR isolation in the last year than in those without. In conclusion, the isolation of MDR pathogen is also high in Non-ICU HAP patients.
