Background: Central airway stenosis often necessitates stent placement, yet the changes in airway microbiota following stenting remain poorly understood. Aim: To study the changes in the airway flora after stent implantation.. Methods: Thirty central airway stenosis patients (age 58.1±10.3 years) were recruited at Emergency General Hospital, China (Jul 2022-Sep 2023). Sixty bronchoalveolar lavage fluid (BALF) samples were collected at four stages: before stenting (BS), with granulation tissue formation (GTF), and without (NGTF). Microbial content was analyzed using metagenomic next-generation sequencing (mNGS). Results: Patients with airway stenosis exhibited significant changes in their airway microbiota following airway stent placement. The quantity of Staphylococcus aureus showed a significant difference before and after placement and was negatively correlated with C-reactive protein (CRP) levels. The abundance of Arachnia propionica differed significantly between benign and malignant airway stenosis. Additionally, after stent placement, during granulation tissue formation, significant differences were observed in the abundance of both Arachnia propionica and Achromobacter ruhlandii in both types of stenosis. Conclusions: Airway stent placement significantly altered microbiota, with notable differences in Staphylococcus aureus, Arachnia propionica, and Achromobacter ruhlandii. erj;66/suppl_69/PA6194/F1 F1 F1
