Background: Central airway stenosis can lead to adverse outcomes for patients. Aim: This retrospective study assesses the efficacy and safety of bronchoscopic contact laser therapy for treating central airway stenosis. Methods: From September 2022 to March 2024, we reviewed patients who underwent bronchoscopic contact laser therapy. Data included age, gender, performance status, initial diagnosis, therapy conditions, symptom improvement, additional interventions, and complications. Results: Twenty-one patients (71.4% male, age 30-70 years, 85.7% with malignant conditions) underwent successful contact laser therapy. Significant improvements were observed in airway stenosis (P<0.001) (Fig 1)and modified Medical Research Council (mMRC) scores (P<0.001). The procedure lasted 1-5 minutes, with minimal blood loss (<20 mL). No complications such as airway perforation, pneumothorax, or significant bleeding were observed. Follow-up bronchoscopy one week later confirmed significant improvements in airway patency without major complications. Conclusion: Bronchoscopic contact laser therapy is a safe and effective interventional treatment for central airway stenosis, demonstrating significant therapeutic outcomes with minimal complications. erj;66/suppl_69/PA6186/F1 F1 F1
