User acceptability of a digital pulmonary rehabilitation (PR) app as an adjunct to usual care

Background: Clinical trials have shown that web-based Digital-PR may be a useful alternative for those who decline outpatient directly supervised PR (Centre-PR); however, limited data exist on the real-world implementation of, and acceptability for, Digital-PR alone or as an adjunct. Aim: To explore acceptability of a smartphone Digital-PR app (Active+me REMOTE) as part of a feasibility study of Hybrid-PR (blending Digital-PR with other PR models). Methods: A qualitative exploratory study comprising semi-structured interviews with fifteen patients with chronic respiratory disease. Data were coded descriptively, and interpretative analysis was facilitated through a Miro virtual whiteboard. The study was carried out by two researchers independent of the app manufacturer. Results: For 2 participants, Digital-PR was a useful adjunct when they may have otherwise dropped out of PR. Reported usage difficulties (user-friendliness was rated low by 6 participants and 3 participants did not use it all) and perceived challenges for others (reported by 6 participants) correlated with poor digital literacy, suggestive of significant barriers to use and adoption. However, 73% of participants stated they would recommend the app to digitally literate friends and family. Conclusions: Willingness to recommend suggests appreciation for Digital-PR. However, poor digital literacy was identified as a significant barrier. This suggests that the potential utility of Digital-PR may be limited to only select digitally literate patient subgroups.
