Hypoxic burden: A novel biomarker in interstitial lung disease (ILD)?

Nocturnal hypoxaemia (NH) is associated with deteriorating quality of life and increased mortality in ILD (Myall KJ et al Chest 2023 Nov;164(5):1232-1242). The concept of “hypoxic burden" (HB) reflects the depth, duration and frequency of oxygen desaturation in sleep better than traditional sleep metrics with greater prognostic significance (Azabarzin A et al EHJ 2019; Apr 7;40(14):1157). Our study aims to explore whether nocturnal hypoxaemia predicts functional decline in fibrotic ILD, as measured by change in 6-minute walk distance. Patients with normal resting oxygen saturations and a minimum of 10% fibrosis on CT chest are eligible for recruitment, with 82 of 160 participants recruited from our ILD centre (London, UK) to date. We present baseline data from sleep studies and lung function tests. Hypoxic burden was calculated using second-by-second sleep oxygen saturation data with the total burden calculated as the sum of areas under the oxygen saturation curve below 90% corrected for total sleep time. Our data shows a weak correlation between HB and % predicted forced vital capacity (-0.304, p=0.03) and oxygen desaturation index (0.338, p=0.01). There is a stronger correlation with mean oxygen saturation (-0.773, p<0.0001) and time spent with oxygen saturation <90% (0.947, p<0.0001). Results from our ongoing longitudinal study will help determine the utility of HB as a novel biomarker in fibrotic ILD. erj;66/suppl_69/PA833/F1 F1 F1
