Dyspnea burden and characterization in the French population: the CONSTANCES Cohort

Despite its clinical importance, the prevalence of dyspnea in the general population remains poorly characterized, with limited data from large-scale cohorts. We aimed to assess the prevalence of dyspnea and identify associated factors in a large French population-based cohort. Among the 205,116 participants enrolled between 2012-2021, 114,346 with data on spirometry, mMRC scale, and smoking history were analyzed. Dyspnea was categorized (0, 1, ≥2) using the mMRC scale. Univariate polynomial logistic regression analyses were performed to study associations between dyspnea and a wide range of clinical, functional (spirometry) and socio-economic factors (ref: mMRC=0). The prevalence of dyspnea (mMRC ≥1) was 21.2% and that of disabling dyspnea (mMRC ≥2) was 9.9%. Significant associations for disabling dyspnea were found with female sex (OR=3.02 [2.89-3.20]), age, current smoking, BMI (BMI≥30kg/m2: OR=7.04 [6.68-7.41], cardio-respiratory diseases (current asthma: OR=2.97[2.82-3.13], COPD: OR=5.69 [4.88-6.63], cardio-vascular diseases: OR=2.11 [2.01-2.21] ), type 2 diabetes, chronic kidney disease, low income and education level, French deprivation index (FDep: most vs. least deprived: OR=1.84 [1.73-1.96])), and poor lung function (FEV1: lowest vs. highest FEV1 quintile: OR=6.49 [6.10-6.95]; FEV1/FVC Dyspnea is frequent in the general population (21%), with nearly one in 10 participants reporting a disabling dyspnea (mMRC≥2), which was markedly associated in univariate analyses with several socio-demographic and chronic conditions. Multivariate analyses will help deciphering the respective contributions of these factors.
