Host-response profiles aid interpretation of rhinoenterovirus detection in adults hospitalised with acute respiratory infection.

Background: PCR detection of Rhinoenterovirus (RV) in adults with acute respiratory infection (ARI) is common, but clinical significance is disputed. Host-response profiling may assess the importance of RV detection. Aim: To compare viral and bacterial host responses using the TriVerity test (Inflammatix, CA, US) in patients with RV. Method: Paxgene RNA serum from inpatients with ARI was tested using Triverty. This measures levels of 29 mRNAs to generate a score (0-50) indicating likelihood of bacterial or viral infection. We compared scores in patients with RV vs other viruses and scores according to clinical diagnosis. Median and IQR values are presented. Results: Of 169 patients tested, 103 (61%) had viruses detected. Those with RV (n=48) had lower viral scores than those with other viruses (27 vs 35) (p=0.018). The RV cohorts (n=34) with co-diagnoses of asthma exacerbation, COPD exacerbation or pneumonia had significantly different viral scores; 35 (31-43), 17(12-33) and 18(11-36), respectively (p=0.016). Corresponding bacterial scores in these groups were 21(14-26), 35(29-38), and 35 (22-41), respectively (p=0.006). Figure 1 shows viral and bacterial scores according to clinical syndrome. Conclusion: RV positive patients had heterogenous viral and bacterial host response scores which differed depending on clinical diagnosis.
