Obstructive sleep apnea, obesity and SCORE2 cardiovascular risk in a middle-aged population - the SCAPIS Sleep Cohort

Study objectives We investigated the prevalence of obstructive sleep apnea (OSA) and obesity and their associations with elevated cardiovascular (CV) risk in the general population. Methods: A total of 9014 participants (age 50-64 years, 46.4% males) from Gothenburg, Uppsala and Umeå in the Swedish CArdioPulmonary bioImage Study (SCAPIS) underwent a home sleep apnea test along with anthropometrics, blood sampling and CT angiography. Mild/moderate /severe OSA was categorized by apnea-hypopnea index 5-14.9, 15-29.9 and ≥30 events/h, respectively. Obesity was defined as BMI >40 kg/m2 or ≥30 kg/m2 with waist-to-hip ratio >0.90 for men and >0.85 for women (Rubino, F. et al. Lancet Diabetes Endocrinol, 2025). Systematic COronary Risk Evaluation 2 (SCORE2) was used to assess 10-year risk of first-onset CV disease. SCORE2 risk ≥10% was classified as a very-high CV risk. Results: The prevalence of mild, moderate, severe OSA and obesity was 35.1%, 9.5%, 3.1%, and 18.4%, respectively. Compared to the non-OSA group, a dose-response association between OSA severity and elevated SCORE2 risk was revealed after controlling for anthropometrics, comorbidities, coronary artery calcification, and hsCRP (odds ratio[95%CI]: mild 1.30 [1.02-1.65], moderate 2.43 [1.79-3.29], severe 2.58 [1.66-3.99], p=0.031, <0.001 and <0.001, respectively). In the same logistic regression model, obesity was associated with increased odds of “very-high CV risk” (1.37 [1.08-1.74], p=0.009). Conclusions: We found high prevalences of both OSA and obesity (new definition) in the middle-aged Swedish population. OSA and obesity should be considered as modifiable risk factors in preventive cardiology. *co-last authors
