Pulmonary rehabilitation (PR) using minimal or specialist equipment:effects on quadriceps strength

Background: PR using minimal equipment (PR-min) may expand the number of settings that can deliver PR. Previous data suggest that PR-min produced benefits in exercise capacity and health status that are non-inferior to PR delivered using specialist gym equipment (PR-gym). However little data exist for resistance training outcomes. Hypothesis: PR-min is non-inferior to PR-gym in improving bilateral knee extension one repetition maximum (1RM). Methods: Participants: Individuals with COPD referred for PR. Intervention:PR comprising 2 supervised sessions per week for 8 weeks using PR-min; n=390. Control:PR-gym. Propensity score matched 1:1 (n=390). Outcomes:1RM, exercise capacity (incremental shuttle walk: ISW) and health status (COPD Assessment Test:CAT). Analysis: Non-inferiority analysis with pre-defined non-inferiority limit for 1RM of 2.5kg. Results: Significant improvements in 1RM, ISW and CAT was seen in both groups, with no differences in Δ1RM between PR-min and PR-gym (Table 1).
