Recruitment of respiratory physicians – disparities between university and non-university hospitals in Germany

Background: Considering the growing prevalence of chronic respiratory diseases, adequate education and recruitment of respiratory physicians is necessary. Methods: Between June and November 2022, the YoungDGP (junior working group of the German Respiratory Society) surveyed respiratory chief physicians (CPs) about their perspectives on physician recruitment at their hospitals in Germany. Results: A total of 53 CPs completed the survey. A threshold of 20% vacant job positions was defined as an indicator of a recruitment problem. This threshold was reached more frequently by non-university hospitals (NUHs) than by university hospitals (UHs), particularly regarding positions for attending physicians (37.5% vs. 5.9%, p = 0.04) and less pronounced regarding resident positions (28.1% vs. 17.6%, p = 0.64). The median perception of recruitment difficulty was higher among NUH CPs for attending positions (75 vs. 10 on a scale from 0-100, p = 0.002). UH CPs reported a higher recruitment difficulty of residents (50) than for attendings, the difficulty was comparable to that perceived at NUHs (56, p = 0.22). Despite not reaching statistical significance, UHs more frequently relied on recruitment of former students/interns (64.7 vs. 40.6 %), whilst NUHs reported more substantial recruitment from external in-country and out-of-country applicants (65.6 vs. 52.4% and 37.5 vs. 11.8%, respectively). Conclusion: These data exemplify disparities between university and non-university medical environments in Germany. Previous data about the perception of respiratory education suggest that a modification of respiratory curricula is necessary to ensure adequate recruitment of respiratory physicians.
