Optimizing inhalation adherence and technique with patient-centric digital technology

Effective management of chronic respiratory diseases strongly depends on adherence to inhalation therapies and proper inhalation technique, yet these factors are frequently suboptimal in daily therapy management. To address these challenges, the PARI Connect system was developed, leveraging patient-centric digital technology. A recent multicenter RCT (Identifier: DRKS00024642) using telemonitoring and coaching in 296 patients with CF, evaluated the efficacy of the PARI Connect app. Results showed a significant 30% increase in adherence to nebulizer therapy in the app group compared to the control group (p<0.005). This improvement was sustained over 18 months, with adherence rates improving by 39% (p<0.005). In addition to enhancing adherence, PARI implemented real-time feedback features into a new companion app to also optimize inhalation technique. The app visually represents the patient’s inhalation pattern, guiding towards a slow and deep inhalation. It provides immediate feedback on inhalation technique and suggests necessary improvements. Initial usability studies (24 participants) indicated that 83% found the real-time feedback helpful or very helpful. First analysis of breathing patterns (25 healthy volunteers, 39 patients with obstructive or restrictive lung diseases) showed a positive impact when using the app. Our findings suggest patient-centric digital technology can improve adherence and inhalation technique. Visualizing adherence can significantly boost adherence rates by 30% and real-time breath-guiding can optimize inhalation technique. The PARI Connect system can support effective inhalation to promote best possible therapeutic outcomes.
