Title: Nationwide trends and characteristics of M. pneumoniae pneumonia epidemics from 2018 to 2024. Background: Macrolide-resistant M. pneumoniae pneumonia (MRMP) is a growing public health concern in Asia. Objectives: The purpose of this study was to investigate national MRMP incidence and epidemiology trends in high-risk age groups from 2018 to 2024 to inform treatment decisions. Methods: The subjects were 34 participating hospitals in the Korean Community-Acquired Pneumonia Study Group (Ko-CAPS) of the Pneumonia and Respiratory Disease Study Group (PRED) under the Korean Academy of Pediatric Allergy and Respiratory Disease. Results: A total of 765 M. pneumoniae pneumonia (MP) cases from 2018 to 2024 were analyzed (mean age: 7.7 ± 4.6 years; 50.2% boys). Annual MP cases were 5.2% (2018), 41.4% (2019), 12.7% (2020), 9.9% (2023), and 30.7% (2024), with no data for 2021–2022 due to COVID-19. Most cases (68.2%) were in children aged 5–12. MRMP increased from 57.5% (2018) to 82.1% (2024) (P<0.005), peaking at 91.7% in June 2024. Fever duration was longer in MRMP (5.7±3.0 days) than in macrolide-sensitive MP (4.3±2.6 days, P<0.005). Use of tetracycline and quinolone rose from 11.2% and 7.0% (2018–2020) to 28.0% and 14.8% (2023–2024) (P<0.005). Chest imaging showed lobar pneumonia in 65.1% and pleural effusion in 8.5% of cases. Conclusions: Continuous monitoring and proactive response are essential to mitigating the burden of nationwide community-acquired pneumonia. To reduce MRMP prevalence, it is essential to actively incorporate alternatives such as tetracyclines or quinolones while continuously monitoring their safety.
