Home-based blood flow restricted exercise in COPD: a randomised controlled feasibility study

Background: Exercise training is beneficial in COPD, but low pulmonary rehabilitation uptake and completion rates highlight the need for more accessible alternatives whilst not comprising effectiveness. Blood flow restriction (BFR) during exercise may enhance the benefits of low-intensity exercise, offering a potential home-based treatment. Aim: To evaluate the feasibility of a novel, home-based BFR exercise program. Methods: Stable COPD patients were randomised 1:1 to five body-weight exercises three times a week for 6 weeks with BFR or without (CON). Feasibility was assessed through recruitment, adherence, and retention rates, and safety via adverse event (AE) monitoring. Acceptability was evaluated through post-intervention surveys and analysis of reasons for declining participation and dropout. Results: 24% of eligible patients consented (Figure 1). Retention was 80%, with 85% adhering to at least 14 of 18 sessions, meeting predefined feasibility criteria. AE were reported in 46% in BFR and 54% in CON, most being non-serious and not directly related to the study. Both groups reported high satisfaction. Barriers included lack of interest, logistical challenges, health concerns, satisfaction with current treatment, and caregiver burden. Conclusion: Home-BFR exercise is feasible and safe for people with COPD. A fully powered randomised controlled trial is required to evaluate its efficacy and long-term benefits.
